Automate image creation - 5000 images

We need 5000 images created - they are identical except for a small text change on each image. We have attached a jpg of the output required and can easily supply the PSD if required. Images need to have the text change (account number) and saved as a individual jpg's - we will supply a spreadsheet of text change requirements (account number) and the image must be save with the account number as the file name. How you achieve the output we require is irrespective, we just need it done be that manually in photoshop, automated through photoshop or whatever method you can use.
