Custom InfoPath Form & SharePoint 2010 Workflow

We have a SharePoint 2010 Enterprise system that we would like to develop a custom workflow for. This workflow will handle user submission and subsequent management of incidents that have negatively affected the business. This solution should utilise InfoPath forms and SharePoint Designer with as little custom code or 3rd party dependencies as possible. Remote access to our SharePoint instance can be provided for deployment and testing if necessary. Please refer to the attached draft requirements document for details.
