Create PDF Template for MonKey Office 2013

I'm using MonKey Office 2013 (not 2014!!) ([login to view URL]) to create invoices and offers. Unfortunately the PDF that I can create does not look like my invoices should look like. So I'm looking for someone who is able to create a custom PDF template for me, that I can use in the application, based on the PDF document that I provide as attachment. It will basically have my companies logo on it, the address and bank details. On certain spots are the variable data displayed like the customers name and address, the date, and the articles and prices I add to the invoice. I need the invoice in german language but I'm able to translate it my self if I have the general code to create the pdf for it in english. Please have a look in the attached PDF file to get an idea how the invoice should look like. The logo will be provided as eps or whatever you need, if required. There rest of the details should come from the application and should be implemented as code variable whenever possible! I'm paying a maximum of $40,- for this job.
