Adjust layout on existing web form

I have a preexisting web form that came with a wifi hotspot package. The form accepts a customer's credit card information. The form works fine, but it's hard to see on a handheld device like an iPhone. Here is what I need: I want the form adjusted so that every field begins on a new line, left justified. For example, instead of Last Name being on the same line as First Name, Last Name is on the line below First Name. This needs to be true for ALL labels/fields. It simply makes it easier to see on a small display if all the entries are on the left. Then the customer doesn't have to pan to the right looking for fields to fill in, and possibly missing some (which is what happens now). Secondly, I want the name of the button "Connect" to instead read "Authorize Payment". See attached files. Let me know if I have included all that's needed. thanx! jim
