Creating an Editable Header while Importing an existing PDF

I'm looking to create a website that allows users to create Professional Headers into existing PDF files. First Step Generic Info: Submitted By Job Name Location Upload Logo (Fields above are to remain optional) Second Step: Then click generate Submittals (The Header will look like something I designed, see attached PDF file named Header) Third Step: customer will now see an 8x11 document with header with editable fields (see attached doc named Header for reference) Customer can fill in more information in the header at this time. Below the Header will be Button that states add PDF. This will allow you to upload an existing PDF file and be placed (Fit to Area) below Header. Please note; if the current PDF File that is being uploaded has more than one page then the header will automatically duplicate and only fill in Generic Information you see in step 1. There should be a button on the left hand side that states Generate New Page with Header (Header should only have the generic information as stated in step 1) and Generate New Page No Header. This will allow the customer to put as many pages in as needed. Fourth Step: Customer can save this in a folder for future reference. Also, if the customer wants to email it may click package and email. Package and Email simply means the website can combine (ZIP) and email to anyone.
