Adobe: Add Signature section and automatic date to intelligent Pdf document

Hi, I have created two intelligent pdf documents for my clients to complete before their treatment. 1. At the bottom, I want them to be able to sign the document with a stylus pen (or their finger) on my Samsung Tablet A. I will then save it to as a secure file. Each patient will need to open the template, complete the questions, sign and then me save the document under their details. 2. I also want the date next to the signature to be automatically today's date. 3. For the consent form, one signature will be the patient and one will be the witness (assistant) I'm offering $20AUD for the tasks. Please quote IVRecovery to indicate you have read the project.
