Hi! The idea here to create a communication between Website-Database-Discord is basically connection your Web to Database for storing of data. Meanwhile you need to have a listener application (can be from your website or different application) which will fetch/listen data updates in your database and from there it will use the Discord API to send messages (like bots do) to specific channels.
