Automated correspondence to communicate with medical professionals

I work as a paediatrician and each patient that I see needs to have a letter sent to the referring doctor. In order to minimise the amount of time I spend on paperwork, I want to be able to tick boxes during the consultation to indicate relevant positives and negatives that I identify. My aim is to tick boxes and type additional comments where necessary during the consultation (as opposed to writing the relevant findings as I discuss with the parent) - this will generate a letter in a separate document that I can send to the referring doctor (likely with some minor corrections). I would need to be able to alter the proforma/database in order to add and delete relevant parts of the consultation process and factor in different conditions that patients present with. Attached is a document about a fictitious patient who has undergone a developmental assessment. The patient has a number of minor medical issues and a presumption of autism spectrum disorder and attention deficit disorder. The letter that this project generates does not need to have this level of detail - but this is generally what I produce when replying to a doctor. I want to be able to keep relevant subject headings in my finished letter, particular the relevant medical issues, medications, my impressions and my plan. Thank you in advance for your help
