I would like a HMTL form creating which i can embed into my website.
The form must allow the following:
Fields to be completed
Documents need to be capable of upload (multiple documents)
Tickbox confirming customers acceptance of terms and conditions and privacy policy. (Must include the link to the terms and conditions)
Once submitted, this form and attachments should be emailed to a set email address.
I have attached a sample and some guidance notes. I am not concerned with the layout so please feel free to set layout however you wish. It just needs to look professional and work.

Hoping the site is in PHP or HTML css we can create the form directly on site and have the data either sent on email or stored in DB.
