Write an iPad Customer Registration application

Hello! What we need is a simple registration form app that runs on Wi-Fi enabled iPads throughout our store. The IPad will collect the following information from the customer via self-registration: 1. First Name *required 2. Last Name *required 3. Email address * or mobile, or both 4. Mobile Number *or email, or both The page will need to have our two logos integrated and look dynamic. Immediately after submitting the form, a promotion code or a coupon needs to be sent to the customer via email and sms. The app should also reset to the registration form so that the next customer can input their information. The developer needs to integrate this app with Square Register in order to directly feed into the customer directory. An API is available with Square in order to accomplish this. This is phase One of the project. Before moving on to phase Two, this app must work seamlessly and efficiently. Phase Two is a separate project that will integrate social media and take advantage of the front facing cameras on the iPad for promotional use. The winner of the phase One bid will be the preferred developer for phase Two if everything in phase One performs well.
