Build me a crossplatform medical app with react native and firebase

I want to create a medical app using React Native and Firebase as the backend. it's a medical application that connects doctors and patients through video, text and audio I have built the UI however, and a workflow and I created it to look a similar to whatsapp Part of the features inculde: 1. Signup for doctors and patients. The logic is quite different though: doctors will have to sign up before logging into the app while patients/clients (I am using these two terms interchangeably) can register after touring the application. 2. Messaging, video and audio. I don't know if you can help build a custom rtc for these things or if we can use an API like sinch 3. The app has a Payment feature which allows doctors to set their price based on if they want clients to - text only for 3 days, text, video and audio for 3 days or text, video and audio for 1 month. We are going to have to use a Nigerian service called Paystack for the payment as Stripe isn't available in the country. They have an api on github 4. A way to send prescriptions and investigations to the client during a chat 5. Users can edit their profiles; especially color scheme for clients 6. Doctor also sees how much he is making per month - Currency is in Naira 7. The backend would have to be able to modify the recommended fee for the doctor to charge and the percentage that the application would earn if the doctor charges above, below or within the recommended prices. 8. It should also be able to calculate how much the doctor has earned based on the number of subscribers/consultations he has had These are the major things for the app, with other ones like searching, inviting contacts, giving feedback, customer requesting questions based on a budget and the app sending these questions requests to doctors who have set their price within that budget.
