Create a Concrete5 website module

I am looking for someone to create a module for a Concrete5 website. Basically we would like a module that mimics PrayBox+ ([login to view URL]) but for Concrete5. It should have all the major features as that product and all requests should be held in the database in their own table. Deliverables to include: 1. source code of module to be uploaded and enable on the Concrete5 website via FTP 2. Support to include the module it up and running properly on the production website once finished.
