Need to create an online registration form page to be added to an existing website from a PDF form The user will need to fill in the required fields in the form and upload 1 photo and there will be mandatory fields. At the end the user will click on submit the form then will be sent to an e-mail address as .csv format

Hi, I have a lot of experience with Joomla. I have been developing sites with Joomla since 2008. I have completed a number of projects here on freelancer.com. I suggest you check my reviews For you project I would recommend the use of a Joomla extension called Seblod. I have designed a lot of forms with Seblod. It would be helpful if you could share the existing pdf as the workload obviously depends on the number of fields and the complexity of the form.
