PHP Assessment/Quiz Site with Template Management

Please see attached for a full description of the project with full specifications. This project consists of a backend template management system to create various assessments/quiz and a front end that will retrieve a template and display the assessment where a user's responses can be saved and later retrieved. No UI design is needed as this module will be integrated into another site in the future. >>> Project needs to be completed within 10 days <<< Full specifications are given so please do not bid if you have no confidence in completing within 10 days.
