10989 ReWork/design sign-up & header

We are looking for someone to re-work/design the sign-up process for customers/agents on our site. There is a PHP script (phpmydirectory) with a mySQL db that is installed and needs to be worked into the process. Also want to re-design the header to make it look cleaner/more professional. We have a site in mind that we have an idea of what we would like it to sort-of look like for both the header as well as the sign-up process which is using the same script. Programmer needs to have HTML, PHP, and mySQL experience.
