This is a very simple, straight forward system to be developed in MS Access in a matter of minutes.
A doctor’s office has a large filing cabinet (see Photo 1) with some 100,000 medical histories covering 50 years of practice, each medical history is numbered with relation to the year the patient first visited the doctor, so each year the numbering restarts at one; recent years end around number three thousand.
When a patient calls to make an appointment, if he/she is not a new patient, the first thing the secretary has to do is find the medical history of the patient; since histories are filed by year and number (and not by name) the secretary looks up the name in a physical card catalog (see Photo 2) which contains index cards (see Photo 3) ordered alphabetically by patient’s name, to obtain the year and history number with which she can then access the large filing cabinet and retrieve the right medical history. The current project’s objective is to computerize just the card catalog, not the filing cabinet.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) A documented installation procedure which may include an actual setup program that will install the software in ready-to-run condition on the platform specified in the Specifications.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
4) A user manual with the basic sections specified in the Specifications.
