ABASE Plugin to build web form and web report

Hi, I want to use the ABASE plugin in my WordPress 4.5 site to have a simple data entry form and search/report page for a Contacts Database. I have created a new My SQL DB on my host and successfully connected it to ABASE. All I need is the data entry form and search/report page. The pages are not for public use, only for office use. This is for us to enter contact and other information, and search and report it later.
