Build a website

Requirements:- 1. can we make the web address unique, no duplicates if i am entering a duplicate it should warn me , but give me an option to overwrite 2. and also can you make sure it cannot be blank, and i want the links for the websites in the data entry tool i should be able to click on them to verify they are working. 3. can we sort them by disease , webaddress and speciality? 4. and also if i only want to view all hematology, records , i should be ale to 5. in front all i need is one text box like google has user can search any thing in there 6. we search our entire database for that word and display results 7. but i need the option to sort them by various specialties or diseases in bank 8. Can search web address also 9. On front end , i need a link to physician registration page Physician First Name, Last Name, Specialty, email address, Phone (optional), Mailing Address (optional) 10. Non- Physician registration also same....Last Name , First Name, Email, Phone and Address Optional 11. Admin can view data of registered member phy. And non-phy 12. Put logo on the corner of reg page link to index page. 13. Need privacy , Terms, about us and contact page. 14. After registration send a welcome mail and contact us mail.
