Convert small amount of php5 code to php7

I have a small amount of code that is written in using php5 msql_ calls i need someone to convert this code to run properly on php7 using msqli calls and what ever else is required to make it run properly. This is working code on older php5 so should not take much to convert. Ive attached the existing code and can provide a populated database if required to test with.
