Responsive Web Form in PHP/javascript

Descriere proiect I need a responsive web form that can be used to populate a contact's information in an address book. This form will need to have all fields in the vCard standard. This includes and but may not be limited to: -Personal Name information, -Address Information with Google Maps autocomplete, and ability to add an unlimited amount of -addresses with different address types -Phone information with intelligent masking and international support -Job title and information -Social network memberships -Instant Message accounts -Important date information - birthdays, anniversaries, hire date, etc -Contact Notes -Contact photo uploader (with crop and resize), cross device functional across mobile, tablet, PC -Relatives (names and relation) -Website URLs -Form Field validation and security protections Please include any information and designs forms you have done in the past. This must be responsive and coded in PHP and javascript / ajax. The final destination will be a MySQL database, but I can handle the implementation of POST, so all form validation must be done in form.
