I am working on a mockup questionarrie for a patient that is visiting hospital. I need a webpage that collects basic data such as first and last name, ,emails and a few questions about patient's health. I will provide the questions. On top of it , there should be a simple cgi script that can read those responses and based on answers to health questions say either its an emergency or not.
