"Quiz" or "Decider" for Web Page - Javascript, HTML, PHP ?

I require one page on my website designed to the following specifications:
-I need a "question" and "answer" format that will result in a product being chosen for the user, based on their answers to the questions.
-This can be built in HTML, Javascript, PHP, or really any format that is universal.
-The questions do not necessarily have to all be displayed at once; that is, one could be answered and the next one appear.
-If no answers are chosen and the button is pressed, an error would be returned. Each question requires at least one answer.
-I am dealing with a fixed height of 840px and a width of 995px.
E.g. of questions:
1) Claim Form of Interest:
[ ] HCFA-1500 [ ] UB-04 [ ] American Dental Association [ ] UCF Pharmacy <----Checkboxes
2) Capability:
[ ] Print (& Mail) Claims

--Since we do not have an Apple version of this product, I would like to include something in the short description saying such.
This probably sounds a bit more difficult than it is. Please let me know if you have any questions. I feel as though if I have a good layout for this, I will be able to tweak it on my own; I just need the start!
