2 simple website forms needed asap

I need a form developer to create 2 separate web inquiry forms. The forms should simply allow a visitor to input their information, and then take them to a thank you page after submission. The forms are very similar with fields such as: Name Address City State Zip Phone Email Employer Occupation 1 Question with 5 choices (They choose 1 of them) 5 Yes/No Questions (They choose Yes or No) The Second will include: Name Address City State Zip Phone Email Employer Occupation and then there are multiple boxes they can choose whichever apply. These results should then be emailed to a set address in a clean format. This project NEEDS TO BE COMPLETED by SUNDAY (8/12/2006) So this project is for 2 forms to be completed. I am hoping to spend $10 to $25 for this project.
