Assignment: To compose a Mood Chart with tracking system for people with bipolar disorder and their supporters (one chart for each one). It should model but not copy these: [login to view URL] [login to view URL] [login to view URL] The bid should include the graphic user interface that is beautiful and looks great, setting it up on our server and all the code. We will provide the text. Elements of Mood Chart: 1. Tracking of medications 2. Tracking of moods (perhaps on a scale of 1 to 10 or levels, as in [login to view URL]) 3. Area for notes on day's entry 4. Journal feature 5. Charts and graphs (printable) Should be able to: 1. Should have simple, user-friendly instructions on how to use (with .pdf file with sample entries and easy-to-understand instructions) 2. Should be simple and easy for person to chart on a daily basis 3. Should be flexible and may be revised in the future 4. Should be printable. 5. Person should have access to chart to print out for their doctor or themselves. 6. Person should be able to edit/update their medications list. 7. Person should be able to access their journal easily. 8. Chart for survivors and chart for supporters should have different URLs and different text. 9. Send daily email to person so that they can chart right from email. 10. Send warning email if they have charted mania or depression for 3 days. 11. Should be able to search it for keywords.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables):
a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
