Coder with PHP/HTML/JS expertise wanted to create and format a survey

I have created a survey structure in Limesurvey. I need someone to add the questions, format the template and add an assessment feature. If you agree to the project I will provide you with: 1) Design graphic with banner for you to create the template 2) Limsurvey file with survey outline with dummy groups and question/sanswers. I will partially complete some of the questions/groups to give you an idea what is needed 3) JS script to enable automatic movement to next group 4) 9 Groups of 20 Questions with 6 Answers each plus one Intro Group (the Intro Group allows the User to select which of the 9 Groups they want to answer, and asks them to rate their ability before they start the questionnaire) You will need to: 1) Download and install Limesurvey on your local server. 2) Read through the help files (they are very good and very extensive) 3) Input the Questionnaire (Groups/Questions/Answers). 4) All Questions use the same simple radio list type question 5) Create a template based on the attached design graphic 6) Add an Assessment feature to the end of the survey (you will I am sure need to research this). This will provide the User with feedack on their score and where they are actually rated compared to the rating they gave themselves before they started 7) Once you have the survey and assessment working you will demonstrate it working on your survey. 8) When we are satisfied you will then send us the survey and template files for us to load onto our servers.
