103123 Need Proposal & Contract Form!

I need a custom, yet very basic form created. This is a proposal/contract form that I will need to be able to fill blanks/spaces and email to clients. It will need to be designed in whatever program is the most popular (easiest for Clients to open! ex. word, works, pdf. whatever the standard is!!) I would also like this to include my logo and possibly an outline border or something to make it look stylish and professional. I am attaching a copy of our logo as well as a basic rough draft of what I need. (The design is basic, however I need the text to be exactly as it reads on the sample). Thanks!!!
