Hi lexmedia, if it is the job just to show some additional fields my bid stays to 30$, if you need to save the data and the data needs error checking, the price doubles at least (saving in the shop database), triples when viewing/editing is required in back-office. I would program a module for you which adds at the bottom of the checkout page with the fields you need. If the data needs to be stored somewhere you need to specify how you would like it.
