VB ASP.NET Web Registration, Large Form, File Upload

This project is to convert a paper Building Permit Application Form into an online form. As it is now, applicants print out the current form, fill it out, and either mail or fax it to us. We then put it into the database through a back end app. What we want to do is cut down the time it takes to do that last step by allowing applicants to fill out the form online, and have that go directly to our database. The database schema is already created and will be given to you. There will need to be a lot of data validation to make sure that valid information gets input. Also, there are documents that are supposed to accompany the form. We would like the users to be able to upload those documents with their application. We would like all of those documents to be in PDF format. If possible, I'd like for the system to be able to convert the docs to PDF automatically. Please include in your bid whether you would be able to do this or not (we will be any extra software that is needed) We would also like the system to automatically calculate the fees due based on the critera laid out in the current paper application, which is attached. We would like for a user to be able to apply for many different permits and be able to come back and see the status of these permits (or come back and upload any missing files, etc) at any time. So that is why we would also like to have a registration and login system. Before a user can apply for a permit, they need to register with the system. So to summarize, * Create a registration and login system * Convert a Paper Form to an Online Form that submits data to a db * Registered users should be able to apply for as many permits as they need and be able to come back and view the status of these permits at any time. * Allow users to upload the required documents that go along with the permit. Those documents need to be converted to PDF. * Each permit should have it's fees automatically calculated. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
