Web survey prototype(repost)

This is to be a prototype or proof-of-concept project that might result in a larger programming contract for a complete product. The project can be conceived of as a web-based survey. For half of the items/questions, the software displays 3 small jpg images with a small button beneath each. When a button is clicked, a border around the indicated image changes color to signal that it has been selected. The user advances to the next item, with a new set of 3 jpg images, by clicking a “Next?? button. The software stores the button that was clicked for each item. Assume that there will be 5 sets of items. For the other half of the items/questions, only one small image will be presented within each item. However, when the item is displayed, a 5-sec _streaming_ audio file is played. Based on a comparison of the information presented in the audio file and the content of the image displayed, users clicks one of three buttons to indicate their decision. Then they click “Next?? to initiate the presentation of the next item. The response selected is stored. Assume there will also be 5 sets of this type of item. The survey will conclude with a brief statistical summary of the responses made displayed in a formatted on-screen table. The data will need to be saved in a MySQL database on the server. The bidder can submit the code using any available images and audio files; they are only place-holders for the materials that will appear in the final product. The project requires delivery of working ASP.NET source code project written using VB.Net 2003 that would sit on the server as well as any components that would need to be downloaded to the client machine. The deadline for the completed working code is September 15, 2005. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform In all 3.0+ versions of Internet Explorer and Netscape.
