Form Mail Clone to Send Data to Specified Addresses Based on Form Field Input

The title is harder to explain than the concept! The concept? We have membership website(s) that collect user data and send that data via email. Our challenge is that we currently get all of the form data into one master email address and we manually have to FWD that email to the proper team member's email depending on the zip code in the email form data that comes to us. We'd like to have this automated! The form should work like FormMail where I can easily implement that across our other sites and can easily format the form / HTML to look nice in the sites. The form should collect the basic information like name and email ... plus ... city / state and zip code as field inputs The form should refer to some kind of database that we enter with the zip codes and the corresponding email addresses that go with that address. We don't have a lot of members, so some kind of flat file would be fine (zip,email) OR if mysql is used, we need some kind of "interface" to add and / or modify the database (I do NOT work directly in that and my staff is very technically unsaavy so some kind of easy interface / admin panel to modify data would be needed IF mysql) When user puts form data in, the script will refer to the database and route the data to the appropriate email address for that zip code. IF THERE IS NO ZIP or a zip is NOT assigned it can go to a "default" address where we'll manually route it if necessary. Also a subject line of "here is a lead from [login to view URL]" would be great for sorting etc. We have had this done before in another situation based upon the state field, but the program was flawed, and we can't find the programmers and we need to start over!
