From to table website

I need a website built using Twitter Bootstrap, PHP and MySQL. Below are the requirements: 1) Navigation: - Home - Submit - Search - Browse - Manage 2) Submit page - User will be presented with a form that I can easily manage the fields for. For now we will say the form will need a) name b) email c) website d) select category - When a user clicks submit, a listing should generate. An admin should be notified of a new submission along with the user. The email to the user should include a DELETE link and a MANAGE link, that will allow them to click and edit/delete their entry at anytime. This link needs to be cryptic so that nobody can just randomly generate the link to delete listings. 3) Search / Browse - A user should be able to search based on category, phrase, or word. - Below the search bar I want each category created listed in bold with the number of entries, like CATEGORY (234) - I want the table of entries to be just that, a table of entries. At the end of the entry I want a "contact" button that will bring up a mobile friendly contact form WITH a captcha. When the user hits send, the email will go to the email submitted by the user. 4) Manage page - In the event a user lost their original email, they will use this page to enter in their email that they used for the listing which will then send them that "welcome email" all over again so that they can edit or delete their entry. This is a pretty simple build and requires minimal design. Most of the work is just getting all of the above to be unified. This will be hosted on a Linux based server using phpmyadmin/MySql.
