Dual Submit PHP form

I have 2 forms that I need process at the same time seamlessly. The first is a mailchimp form we're hosting our on site. The form is processed and then mailchimp takes them to the confirmation page. However, I want the data to also be submitted to another database, so I need one single form, and the data going to two places. I don't need any design work, just the raw code and script working perfectly. I will have my developers migrate it to live on our site. Lastly, it is important to note that the fields are both, first name and email, however they are called something different in each database. One of them is something like "Fname" and the other is something like First_Name so, be sure that you can do this. Will award best qualified, and fairly priced individual on Monday AM. We are a marketing agency in the USA, so bid accordingly. Should take someone who knows PHP only an hour or 2. Lots of tutorials on this. In your text bid, confirm you can do this.
