270680 vTiger Activity form

I hyave created a report in Crystal Reports which provides a form from the calendar record in vTiger. While Crystal created the form quickly and easily; it cannot integrate easily with vTiger. I require any logged user to be able to produce this form from a calendar lookup table if possible. Attached detail shows an example of the Crystal Report form. The yellow shaded fields are from the activity/contactdetails/cf tables The rest of the form is text, lines and boxes. All offers considered
