Combine Tell A Friend with Comments

I want a script that opens a 'tell a friend' window with: Name Recipient Email From Sender's Email Friends and Family Email Message Box THEN I want the script to post the name of the recipient and the message in a comments field for everyone to see with only the most recent comments showing. It needs to have bad language controls. I have seen both these scripts for free, but don't know how to combine them. I will need instructions how and where to add them to my site and change and text I want. My provider includes mysql, perl, etc. so use what is easiest. This seems quick and easy. So basically i want a pop up email window that also populates to a "Comments" area of my page.
