Contact form and records into Database

I need a simple contact form? that not only sends us the email but also records the contact info into a mySQL that also has an administration side. ***I do not care if you didn't make it all from scratch you can use scripts. IT JUST HAS TO WORK HOW I WANT IT!*** ? Form ------------------------------- The Contact needs to consist of 4 fields? and "*"? means required: *name **email (if no email then phone number) phone # *comment Please note that if they do not have an email for contact they MUST enter a phone number. So its one or the other or both. Comment needs to be a large box. mySQL --------------------------------- I want all the info to be saved onto a DB. Everything including comment. (If adding the comment part, say they write a lot, might make it unstable let me know) Administrative page ---------------------------------- I want something like: <[login to view URL]> or <[login to view URL]>? ? ? for the URL. When you go to that just a simple form for username and password. In the page: 1) I want to be able to see all the things saved on the database. IN AN ORGANIZED FASION. 2) Able to send a mass email to all the email's saved. This section of the admin page should have only Subject and Comment area and a way I can add something like [name] in the comment area to address each person. 3)Change password for admin page Example: Dear [name], would show them Dear John Smith, (retrieved from the DB table per email) ## Deliverables If you do not understand what I need message me and I will further explain.
