Email/Database script needs fixed on web site - PHP/MySQL

I have a membership website where the members fills out forms to request information. The admin then pulls up the record(which is in a form) and enters 2-3 more fields of data than clicks an "update" button to send that member an email with the new data..(plus it adds the data to the database). The email function is not working. This is what needs fixed.
