Priceline like website model

My website is currently up and operational. Currently written in php based on a linux server. I need someone who can overhaul the website making it more user friendly and help retouch/ retool a couple of forms. We are also looking to add a new feature, which will incorporate a online registration service with calendar booking. We will need 3 sides to this feature, a customer interface, a client interface and of course an overall management portal for us. We also use online maps to show locations for certain facilities we use. I will need a map that automatically populates with location information based upon an online form that is filled out. you will help create these forms. This is a priority project for me with a deadline of 1 month. I will not waste your time so please do not waste mine.
