530238 Wordpress / Contact form 7 form 3

Hi All, I have a wordpress site for a client and it has a contact form 7 form that you can see here: [login to view URL] I need the fields to display in the right place. some of the fields are not positioned right. Also i need the email that is send after submitting to change. I want the same layout from the form to be also in the email that is send. so there needs to be created a full email layout for this form! (because the standard email is very hard to make up what options are selected) Give me you best bid. i need this asap
