(Wordpress plugin) Custom form and display

I need a form created that will allow our employees to generate workouts for their clients. I need to have the ability to insert these forms wherever I need via shortcode ([form id=1], [form id=2], etc.). I also need to display the submitted data via shortcode ([formsub id=1, [formsub id=2], etc.). On the admin side of things, I need an area to create categories, and the ability to add exercises to those categories. Those exercises need to also have an area in which I can create, add details, and include youtube video(s). Only exercises I have created can be added to a category. The directions for the form and displays are in the attached photos. Please review and have a good understanding of this project before bidding. I am more than willing to be contacted via skype and email in order to discuss the project to get it right. In fact I prefer it rather than seeing something not done right various times. *** Because this project is under a deadline, I will offer 100% payment if the project is completed within 7 days of acceptance. every day thereafter, there will be payment penalties (3.5% of bid price per day).
