Provide urls of every single page that is tested. Include screenshots of sections that are tested. In a workflow such as buying a ticket, provide all urls and screenshots of everything. Add notes for anything that seems like a bug, issue or error. Include the browser and os that was used. The first run should be a desktop site test, unless outlined below.
