edit form data on multiple forms so they all function on the same page

I am sure this is really simple for you guys. I have copied and pasted several forms that I use on different pages, and have tried to make them all function on the same page. Problem is when I have copied and pasted them the field ID has numbered itself making the form data inactive. Please see that attached file for a better understanding Right now the top left data field is the only one that is working. I need them all to work on their own SUBMIT buttons as they all have different form data and will yield a different result Thanks
